• Finding and transitioning to assisted living or nursing home facilities.
• Navigating insurance and financial assistance options including research to support Medicare. Advantage plans selection
• Monitoring overall health and safety to address concerns proactively.

3. Coordination of Services

Our Patient Advocates can coordinate and manage as much of your loved ones healthcare as you want, including

• Working with all of your loved one's providers to ensure their care is coordinated, communicating findings and next steps directly with the family.
• Taking care of administrative tasks like scheduling medical appointments, securing transportation or tracking medication refills.
• Reviewing and managing medical bills.

5. Being there when you can't

Patient advocates can also be on the ground with your senior for emergencies or if you live far away. This includes:

• Attending medical appointments to communicate needs and ensure quality care.
• Being present at the hospital to ensure staff have the correct medication list, know your parent's medical history and are being responsive to their needs.
• Coming to your senior loved ones side in a medical emergency to ensure good care until you can arrive
• Overseeing and ensuring quality from home healthcare services, assisted living facilities, nursing homes or dementia care units.
• Reminding your senior about doctor's appointments.
• Ensuring compliance with treatment plans including medication management.
